Audio transcription of in-depth interviews for Choice Architecture Based TB Preventive Therapy Prescribing (CAT) studyKNCV Tuberculosis FoundationKNCV Tuberculosis Foundation (KNCV) is one of the leading international non-profit organizations dedicated to fighting tuberculosis (TB) worldwide. We are an international center of expertise for TB control that stimulates effective, efficient, and sustainable tuberculosis control strategies in a national and international context. We are an organization of passionate TB professionals, including medical doctors, researchers, training experts, nurses, and epidemiologists. Over the past century we have built a wealth of knowledge and expertise, initially by successfully driving back TB in the Netherlands, and subsequently by partnering in the fight against TB in over 40 countries worldwide. We operate from a central office in The Hague in the Netherlands and additional country offices worldwide. KNCV raises funds from individual institutional, and corporate donors.BackgroundKNCV with financial support from Unitaid is implementing a cluster randomized trial of a clinic-level implementation strategy for TB Preventive Therapy (TPT) delivery to people living with HIV (PLHIV) in Mzimba North and Mulanje districts in Malawi – the CAT study. A choice architecture strategy is used which makes prescribing TPT the “default” or standard option while for TPT that is not prescribed the clinician is required to make the choice to “opt-out” of TPT for a specific patient. This is implemented through the national HIV EMR by either switching off the default prescribing module (control sites) or switching on the default prescribing module (intervention sites) to make TPT part of ART prescribing or re- prescribing. The primary objective of the study is to test whether this choice architecture approach of “default” TPT prescribing when prescribing ART substantially increases 3HP/IPT prescribing. Nineteen clinics in Mulanje and Mzimba North are participating in the CAT study and were randomized to be either an intervention or control site. There are 10 control and 9 intervention sites. The study officially started follow-up in the sites on 1st April 2021. Monthly data collection and two fidelity assessments are conducted to observe the use of the EMR by providers in the sites.Purpose of the consultancyAs part of the CAT study, in-depth interviews are conducted to assess whether the choice architecture “default” TPT prescribing approach when prescribing ART substantially increases 3HP/IPT prescribing.
