Licensed Practical Nurse Care Manager

LPN Care Manager Duities:
• Patient Assessment: Conduct thorough assessments of patients' medical, social, and emotional needs to develop individualized care plans.
• Care Planning: Collaborate with interdisciplinary team members to develop comprehensive care plans that address patients' physical, emotional, and social needs.
• Care Coordination: Coordinate healthcare services, appointments, and referrals to ensure timely access to medical treatments, therapies, and support services.
• Medication Management: Administer medications, monitor responses to treatment, and educate patients and caregivers on medication regimens and side effects.
• Patient Education: Provide patient and family education on disease management, treatment options, and strategies for promoting health and wellness.
• Documentation: Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in electronic health records (EHR) systems.
• Communication: Communicate effectively with patients, families, caregivers, and healthcare team members to facilitate coordination of care and promote continuity of services.
• Quality Improvement: Participate in quality improvement initiatives, audits, and performance improvement activities to enhance the delivery of patient care and services.
LPN Care Manager Requirements:
• Education: Graduation from an accredited Licensed Practical Nurse (LPN) program.
• Licensure: Current, unrestricted LPN license in the state of practice.
• Experience: Previous experience in care management, case management, or a related healthcare field is preferred.
• Clinical Skills: Proficient clinical assessment skills and knowledge of medical terminology, disease processes, and treatment modalities.
• Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, families, caregivers, and healthcare team members.
• Organizational Skills: Strong organizational and time management skills to prioritize tasks and manage multiple patient cases concurrently.
• Computer Skills: Basic proficiency in using electronic health records (EHR) systems and other healthcare software applications.
• Compassion: Compassionate and empathetic approach to patient care, demonstrating sensitivity to patients' cultural, emotional, and social needs.
